John M. Bockrath, MD
Dr. John M. Bockrath earned his medical degree at Northwestern University Medical School in Chicago, Illinois in 1976. He completed his general surgery and urology training six years later at Northwestern University Medical Center. He has been a clinical instructor in urology at Northwestern in the past. He was a recipient of an American Urological Association Award for Laboratory Research in 1981. He has authored several medical publications. Dr. Bockrath practices adult and pediatric urology with special interests in urologic oncology, urinary incontinence, erectile dysfunction, kidney stones, prostate diseases, male infertility, and vasectomy reversals.
He is certified by the American Board of Urology and a Fellow of the American College of Surgeons. He is a member of the Chicago Urological Society,
American Urological Association, and DuPage County Medical Society. Dr. Bockrath is a member of the Department of Surgery at Hinsdale Hospital, Edward Hospital in Naperville, and Advocate Good Samaritan Hospital in Downer's Grove. He has been an avid lecturer and has been published in several prestigious medical journals including the Journal of Urology.

Robert M. Pasciak, MD
Dr. Robert M. Pasciak is a graduate of the Loyola University Stritch School of Medicine where he completed his internship in general surgery. After four years of military service in the United States Navy, Dr. Pasciak returned to Chicago and completed his urology residency at Northwestern University and Childrens' Memorial Hospital. He has been in clinical practice in the western suburbs while holding an appointment at Northwestern as a clinical instructor for several years.
He is certified by the American Board of Urology and a Fellow of the American College of Surgeons. He is a member of the Chicago Urological Society,
American Urological Association, and DuPage County Medical Society. Dr. Pasciak is a member of the Department of Surgery at Hinsdale Hospital, Edward Hospital in Naperville, and Advocate Good Samaritan Hospital in Downer's Grove.
Dr. Pasciak has authored several medical publications and has participated as a clinical investigator in the Nuclear Matrix Protein Study Group for Bladder Cancer. He practices adult and general pediatric urology with special interests and skills in urologic oncology, prostate disorders, continent urinary diversions, kidney stones, incontinence, and vasectomy reversals.

Paul B. Lyon, MD
Dr. Paul B. Lyon received his undergraduate and medical degrees from the University of Chicago. He completed his post-graduate medical studies at the University of Iowa Hospitals and Clinics in Iowa City. Dr. Lyon was certified by the American Board of Urology in 1997. He is a member of the American Medical Association,
American Urological Association, Chicago Urologic Association and DuPage County Medical Society.
Dr. Lyon was the recipient of a
National Institutes of Health Service Award training grant in 1992-1993. He is a member of the American Medical Association, American Urological Association, Chicago Urologic Association and DuPage County Medical Society. He is on staff at Edward Hospital, Good Samaritan Hospital and Hinsdale Hospital.
Dr. Lyons post-graduate training was in brachytherapy, pathology, uroradiology, transurethral thermotherapy and laparoscopy. Since 1996 he has given instructional presentions on the topics of:
- Brachytherapy for prostate cancer
- Using the Prostate Specific Antigen Test
- Management of urinary calculi
- Evaluation of hematuria
- The overactive bladder

Bejan J. Fakouri, MD
Dr. Bejan J. Fakouri is certified by the American Board of Urology. He received his medical degree from the Indiana University School of Medicine. He completed his internship and residency at Rush Presybyterian St. Lukes Medical Center in Chicago where he was chief urology resident from 1998 to 1999. Dr. Fakouri is a member of the
American Urological Association, American Association of Clinical Urologists, and DuPage County Medical Society.
He has authored over a dozen articles concerning male infertility, benign prostatic hyperplasia, testis cancer and neurogenic bladder.

Robert M. Seo, MD
Dr. Robert M. Seo received his undergraduate degree from Yale University and his medical degree from Columbia University College of Physicians and Surgeons in New York City. He came to Chicago for his post graduate training and completed his general surgery and urology training at the Northwestern University Medical Center. He then completed a fellowship in male reproductive medicine and surgery at the University of Illinois at Chicago. Dr. Seo practiced in Virginia prior to returining to Chicago. He practices all aspects of urology and he has a special interest in male infertility.
Dr. Seo is certified by the American Board of Urology and is a member of the American Urological Association, American Society for Reproductive Medicine, Chicago Urological Association, and DuPage County Medical Society. He is on staff at Edward Hospital, Good Samaritan Hostial, and Hinsdale Hospital.
Dr. Seo has authored numerous articles and has given presentations on topics including male infertility, urological infections, and prostate development.
